Health Forms Hold

Processes affected
  • Registration actions (add/drop/withdraw)
Steps to have hold lifted
  1. Log in to the Patient Portal.
  2. Go to the Medical Clearances tab. 
  3. Find the Health History and TB Questionnaire lines.
  4. Click "update" and complete both forms. (Cascades campus students only need to complete the TB Questionnaire form). 
  5. Once both forms are completed, the hold will be removed once the next automated report runs (runs daily at 10 AM, 1 PM, 4 PM, 9 PM).
